QIDP Case Manager

Job Description:

Responsibilities:
  • Ensures all areas of responsibility are in compliance
  • Advocates for services and supports to help the individual and their guardian/representative achieve their outcomes
  • Serves as primary contact person for guardians and all service providers
  • Ensures that all additional assessments are completed in a timely manner
  • Develops a Service Agreement that incorporates all needed services for the individual
  • Trains families, guardians and personal service workers on implementation of the service agreements
  • Completes written monthly review of the individual’s Chronological log and submits complete hours of service tally to director in a timely manner
  • Assures that all services are delivered as outlined in the service agreement
  • Completes home visits in person or virtual as required
  • Meets State of Illinois guidelines for annual QIDP certification.
  • Completes documentation reviews as required by Quality Assurance
  • Advocating, communicating and facilitating effectively
Desired Skills:
  • Strong interpersonal and communication skills
  • Ability to work effectively as part of a team
  • Knowledge of behavior management techniques and strategies
  • Experience working with individuals with disabilities at least two years
  • Ability to utilize Therap
  • Willingness to learn and grow with the organization
  • Someone who has experience as a QIDP

  • Develop and supervise employee(s) within their team and oversee services being provided at the Center of Hope

Requirements:
  • Must have a Bachelor’s Degree in the Human Services field, including but not limited to Sociology, Special Education, Rehabilitation, Counseling, or Psychology
  • Must have a Valid Driver’s License
  • Must be able to pass a background check
