Transition Services

Self-Representation Guardian Support
We assist families who are representing themselves in the guardianship process by providing guidance on completing required paperwork, explaining court procedures, and helping them understand each step of the process. Our goal is to equip families with the information and support they need to confidently navigate the court system.

Bridges School Transitioning
Bridges is a program to assist families and professionals in transitioning students with developmental disabilities to the world of adult services. Our Transition Outreach Specialist helps families find resources for: enrolling children on the PUNS list (Prioritization for Urgency of Need for Services), Financial/Medical benefits, Legal Resources, Program Funding (Home & Community Based), Waiver Programs, Training/Coaching, Recreational Activities, and attending Individualized Education Plan (IEP) meetings to ensure transition is part of the IEP.

Self-Directed Assistance
We support participants and families with support and opportunities within their home and community to increase participants’ independence. Our SDA services provide guidance with person-centered planning, budget management, service coordination, recordkeeping, and recruiting Personal Support Workers (PSWs). We use a strength-based approach to individualize everyone’s goals and preferences, empowering participants to learn and generalize independent living skills, make informed decisions, and achieve greater independence.
Community Day Services

Cultural Arts
Cultural Arts activities provide opportunities for adults with intellectual and developmental disabilities to explore creativity through visual and performing arts. Activities may include various art and music options. These activities support self-expression, fine motor skill development, emotional well-being, and social interaction.

Sports Class
Sports Class offers adaptive physical activities and games designed to improve coordination, strength, teamwork, and confidence. Activities are modified to meet individual abilities and encourage participation and enjoyment of movement.

Memory Lane
Assisting our senior participants, and those experiencing dementia or Alzheimer’s, age with grace and help maintain their current skill sets.

Book Club
Book Club encourages literacy, listening, and group discussion through books, short stories, or audiobooks. Activities support communication skills, comprehension, social interaction, and enjoyment of shared reading experiences.

Technology Class
Technology Class teaches basic computer skills such as typing, navigating programs, and safe internet use. Participants may also be introduced to 3D printing concepts, supporting creativity, problem solving, and digital literacy.

Community Experiences
Community Experiences provide opportunities for participants to engage in their local community through outings such as libraries, stores, parks, and community events. Activities focus on building independence, communication skills, community awareness, and safety.

Plant Class
Plant Class involves hands-on gardening and plant care activities. Participants learn about planting, watering, and caring for plants while developing responsibility, patience, fine motor skills, and an appreciation for nature.

Community Employment Services
Community Employment Services (CES) assists individuals who are able to work or volunteer out in the community.

Cooking Class
Cooking Class focuses on teaching basic meal preparation skills, kitchen safety, and nutrition. Participants practice following simple recipes, using kitchen tools safely, and developing independence in daily living skills.
